Wound Care Director - Full-Time

Siskin Hospital for Physical Rehabilitation is seeking to add a full-time Wound Care Director to our award-winning team of medical professionals. Our Wound Care Director directly supervises departmental staff. He/She will design and implement programs to improve patient care and outcomes, coordinate and evaluate the care of patients with wounds, ostomies, and continence issues. They provide direct nursing care to patients with fistulas, pressure injuries, venous and arterial ulcers, incontinence and other skin and wound problems while providing staff, family and patient education on skin integrity and wound related issues. The Wound Care Director uses evidence-based practice to develop care plans and uses the skin integrity policy to guide treatment planning. The Wound Care Director collaborates with the Therapy and Medical teams to coordinate patient care and direct outcomes. He or She monitors wound healing progress and evaluates the need for treatment changes while accurately documenting wound assessment findings, interventions, referrals, recommendations, and wound healing status. He/She coordinates QAPI efforts for the Wound Care Department.

 

Under the direction of the VP of Patient Care Services/CNO, the Wound Care Director has the responsibility to provide direct and indirect care for rehabilitation patients; to provide guidance to students as needed; to monitor supply usage; to initiate emergency procedures related to patients, families, staff, and institution; and to adhere to all criteria set forth by CIHQ, CARF and the Tennessee State Board of Nursing.

 

PATIENT AGES SERVED: Adolescent (12-18 years of age), Adult (19-65 years of age), & Older Adult (over 65 years of age)

MINIMUM QUALIFICATIONS OR EQUIVALENTS:

  • EDUCATION: Bachelor’s degree in Nursing

  • EXPERIENCE: Three years of experience in wound care in an acute or sub-acute setting

  • LICENSURE/CERTIFICATION:

  • Current Tennessee State RN licensure

  • Current CPR certification.

  • CWOCN preferred. Must obtain within one year
