The SubAcute Rehabilitation Program offers personalized skilled nursing care in Tennessee’s largest acute rehabilitation hospital. Our SubAcute Rehabilitation Program at Siskin Hospital West specializes in short-term rehabilitation at the skilled and SubAcute level, with a commitment to returning you to the community at your highest possible level of function and independence.
The SubAcute Program is a unique program designed to provide patients with an interdisciplinary therapy program that is less intensive than traditional comprehensive inpatient treatment.
Providing exceptional nursing and therapy care is our top priority. As a higher-functioning skilled nursing unit, you will get the rehabilitation care you need to return to an active lifestyle.

Subacute West is located in the copper building. Turn left before you enter the main hospital drive. Parking can be found under the building.
Visitation is allowed for any patient that has been admitted for more than 14 days. Visitation days/times are currently:
Monday – Friday, 4:00 – 7:30 p.m.
Saturday – Sunday, 12:00 – 7:30 p.m.
