MDS Coordinator (full-time)
Are you a Tennessee-licensed RN or LPN that has experience in long-term care, with specific experience in MDS coordination? Siskin Hospital for Physical Rehabilitation is currently seeking a Full Time MDS Coordinator for our Skilled Nursing Facility.
This position reports to the Administrator and/or Director of Nursing and will be responsible for providing an ongoing objective evaluation of the important aspects of patients’ care through review of patient charts, identification of variations in care to provide data for use in the correction of potential problems, and to meet the requirements of all regulatory agencies.
The successful candidate will relate effectively with interdepartmental and intradepartmental personnel, medical staff, patients, patient’s families, and outside agencies. They will conduct and coordinate the development and completion of the resident assessment in accordance with the state's requirements and this facility's policies and goals.

MINIMUM QUALIFICATIONS -
Experience:
- Minimum of 1-2 years of experience in long-term care, with specific experience in MDS coordination or similar roles preferred. 
Licensure / Certification:
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with an active nursing license in the state of TN. 
- BLS Certification. 
- Certification in MDS (e.g., Resident Assessment Coordinator – Certified (RAC-CT)) is highly desirable. 
Skills:
A. MDS
- Strong knowledge of MDS 3.0 guidelines, OBRA, and PPS. 
- Excellent organizational and time management skills. 
- Proficiency in electronic health records (EHR) and MDS software. 
- Strong analytical and problem-solving abilities. 
- Effective communication and interpersonal skills. 
- Ability to work independently and collaboratively within a team. 
- Strong clerical skills with ability to type. 
- Computer knowledge literacy and ability to operate. 
- Excellent interpersonal and telephone etiquette skills. 
- Self-directed, ability to manage various responsibilities in a calm, effective manner. 
- Strong organizational skills 
B. LPN/RN
- Working Knowledge of Patient observation/assessment (vital signs, neurological signs, skin, bowel, etc.) and medication administration.
