Inpatient Case Manager

Siskin Hospital for Physical Rehabilitation is seeking a dedicated Inpatient Case Manager to join our team and play a crucial role in guiding patients through their rehabilitation process. This full-time position offers the opportunity to work directly with patients and their families, coordinating care, developing treatment plans, and ensuring that every patient has the support they need to return home with the best possible outcomes.
As an Inpatient Case Manager, you will collaborate closely with healthcare teams, patients, caregivers, and external organizations to provide comprehensive care and support. You'll be the primary point of contact for patients and their families, orienting them to the rehabilitation process, monitoring progress toward goals, and ensuring a smooth transition from inpatient care to home or further treatment. Your role will have a direct, positive impact on both the recovery journey of patients and the efficiency of their discharge planning.

Key Responsibilities:
Conduct initial patient evaluations and provide an overview of their rehabilitation program.
Develop individualized treatment plans with the goal of helping patients return home.
Monitor patient and family progress toward short- and long-term rehab goals, ensuring all necessary care and services are in place for discharge.
Coordinate Durable Medical Equipment (DME), medical equipment, and follow-up services before discharge.
Educate patients and caregivers on available follow-up therapy services and make referrals as needed.
Advocate for patients, ensuring they are actively involved in their rehabilitation journey.
Ensure the collection of data for PAI documentation and contribute to the discharge planning process.
Qualifications:
Education: Minimum of an Associate's Degree in healthcare or a related field (preferred)
Experience: At least 1 year of experience in healthcare, ideally in a hospital or rehabilitation setting (inpatient hospital experience preferred)
Skills:
Strong communication and interpersonal skills to work effectively with patients, families, and healthcare teams
Knowledge of rehabilitation processes and discharge planning
Ability to manage multiple tasks and responsibilities in a fast-paced environment
