TN Yellow DOT Program

The Tennessee Yellow DOT Program is designed to provide first responders with your medical information in the event of an emergency on Tennessee’s roadways.

This information can mean the difference between life and death in the “Golden Hour” immediately following a serious incident. 

Participants in the program receive a Yellow DOT decal, a Yellow DOT folder and a Medical Information Sheet. You can place your photo on the sheet for identification purposes. You complete the Medical Information Sheet with your emergency contact information, medical information, recent surgeries, hospital preferences, current medications, insurance and physicians’ information. This information will be your sole responsibility and should remain in the glove compartment inside the yellow folder provided.


The Yellow DOT decal will be placed on the driver’s side rear window of their vehicle.

It’s simple! Pick up your packet at one of the locations listed below. Complete the Medical Information Form, enclose your photo, and mail to the designated address.


You can pick up the packets at the following locations:

Siskin Hospital for Physical Rehabilitation’s Main Campus

Packets are available at the reception desk at the main entrance:

  • Monday-Friday, 4:00 p.m. to 7:30 p.m., Monday through Friday
  • Saturday-Sunday, 12:00 p.m. to 7:30 p.m.
