Inpatient Case Manager
A case manager is assigned to individual patients when they are admitted to an Inpatient Rehabilitation Program. As the primary coordinator of the provision of care, case managers complete initial evaluations and assess the caregiver’s ability to assist the patient upon discharge from Siskin Hospital.
The case manager ensures communication between the treatment team, patient, and caregivers with regards to treatment plan, goals, availability of community resources, and outcomes of care. They ensure that all medical equipment and follow-up services are arranged prior to the patient’s discharge from Siskin Hospital.
The case manager will make arrangements for the patient to be transferred to alternative levels of care when discharging to their home is not an option. They coordinate family training sessions and act as a patient advocate during the patient’s stay at Siskin Hospital.
Siskin Hospital's case management team include a Licensed Bachelor Social Worker (LBSW), a Certified Case Manager (ACM), and a Certified Brain Injury Specialist (CBIS).
