Other Important Information

     

    Home Health Patients

    Outpatient Services are non-covered for Medicare patients enrolled in Home Health Services (if the Home Health Agency bills Medicare, it qualifies as Home Health).

     

    Options for those patients are:

    1. These patients may sign a Medicare Waiver agreeing to pay for the services.
    2. The Home Health Agency may sign an agreement with Siskin Hospital agreeing to cover the therapy charges.
    3. The patient may be discharged from Home Health until Outpatient Therapy services conclude.

     

    Physician Certification of Plans of Care

    A "certified" plan of care (meaning signed by the Physician) is required to meet state and federal regulations, as well as to maintain our certification status. Reimbursement for your services is also tied to a timely certification of the plans of care. For this reason, we ask your cooperation in obtaining timely physician signatures on all plans of care submitted to your office. Another physician in the office may sign plans of care if the ordering physician is unavailable. Please note Medicare requires a re-certification every 30 days, therefore you will receive multiple requests for signature on patients treated longer than 30 days.

     

     

    - Orders for Service

    Our policy requires the patient have an order for service written within the past 30 days. We will contact your office for an updated order, should a patient seek therapy past the 30-day limit.
