Liaison Instructions for Inpatient Referrals to Siskin Hospital![]()
We appreciate your patient referrals to Siskin Hospital for Physical Rehabilitation. The following guide will assist our nurse liaisons in processing referrals for our inpatient services. Siskin Hospital employs six full-time nurse liaisons assigned to assist each of the area hospitals with the admission process.

1. Fax or Call Your Referral to Siskin Hospital's Admissions Department

To call: Call the Inpatient Admissions Department at (423) 634-1277.
2. Required Information for Referrals
a. Name of Patient
b. Diagnosis
c. Referring Facility's Name
d. Referring Facility Case Manager's Name
e. Face sheet (demographic and all insurance information)
f. Family Contact (name and phone number)
g. Patient's Room Number
h. Primary M.D. (name and phone number)
i. Referring M.D. (name and phone number-if not the primary M.D.)
j. History and Physical (primary consults)
k. Most Recent Labs (CDC, Metabolic, Coagulation, any others)
l. All Cultures and Results
m. Vital Signs, Current and Recent Trends (last 2-3 days)
n. Current Medication List (including intravenous medications)
o. Initial Therapy Evaluations and Current Progress from PT, OT, and ST
p. Dietary Consults (if done)
q. M.D. Progress Notes (last 5 days)
r. Latest Chest X-ray, MRI, CT Scan
s. Any Other Pertinent Diagnostic Testing
3. Approval Process
All patient admissions must be medically approved by Siskin Hospital's Medical Director and financially approved by our Finance Department.
4. Final Arrangements
a. Ambulance arrangements may be made through the Admissions Department.
b. Once we have received approval for admission, the patient must arrive at Siskin Hospital no later than 2:00 p.m. on the day of admission. A family member needs to accompany the patient so they can sign them into Siskin Hospital.
