Admissions

    Following a physician's referral, our Nurse Liaisons will review patients' medical records and then meet with the patient and family to determine if the patient's needs are a good match for our rehabilitation program.

     

     

    General guidelines for admission include the need for patients to be:

    • Neurologically and medically stable
    • Free of ventilator support
    • Able to tolerate active participation in therapy
    • Without an active seizure history in the past 96 hours or on a successful treatment program of anti-convulsants

     

    Your Nurse Liaison will then coordinate your admission to Siskin Hospital.

     

    Your transportation from another facility to Siskin Hospital is arranged by your current case manager. You and your family are only responsible for transportation to the hospital if you are admitted from home. When you arrive, we will provide bedside admission and a patient information folder to answer more detailed questions about your time here.
